Providing Accurate Information

  • Provide honest, complete, and accurate information about your concerns, history, medications, and relevant life circumstances.

  • Inform your psychologist of any changes in your mental health, risk status, or medication prescribed by other providers.

Engagement in Therapy

  • Actively participate in the therapeutic process.

  • Inform your psychologist if you do not understand your formulation, treatment plan, or recommendations.

  • Acknowledge that therapy requires effort and collaboration.

Respectful Conduct

  • Treat psychologists, administrative staff, and other clients with respect.

  • Maintain appropriate boundaries and professional communication.

Appointments and Cancellations

  • Attend sessions on time.

  • Provide adequate notice if you need to cancel or reschedule, according to clinic policy.

Financial Responsibility

  • Fulfill agreed payment obligations in a timely manner.

  • Clarify insurance coverage independently if applicable.

Clinic Policies and Safety

  • Comply with clinic policies and procedures.

  • Maintain safety and appropriate behavior within the premises.

  • Safeguard your personal belongings while on clinic premises.

Personal Responsibility for Decisions

  • Understand that you retain full responsibility for personal decisions made during or after therapy.

  • Acknowledge that choosing not to follow therapeutic recommendations may influence outcomes.
